What is the CareChoice Cooperative?

CareChoice is the nation’s first cooperative made up of not-for-profit, mission-driven providers of aging services. Organized in 1996, the members of the cooperative came together primarily for the purpose of contracting with managed care entities for skilled nursing services. Today, CareChoice has managed care contracts with all of the major health care systems. We also look for other opportunities to better serve our residents.

Our mission: To provide financial and operational benefit(s) to our member organizations through innovation and collaboration.
